I have moved out of state and lost my Connecticut license. What can I do? 
You may contact us through our Telephone Center or by e-mail mail@ct.gov with your name, date of birth, and former Connecticut address. If a mailing address is provided we will mail you a verification letter or if a fax number for an out-of-state DMV is provided we will fax the letter verifying your Connecticut license.

I need my driver history. What do I do?  
Please provide your name and current mailing address and we will mail you the required Copy Records Request (form J-23). You will need to complete section 1 of the form and return it with the required fee of $20. You must also include copies of two forms of identification as stated on the J-23 form. Your history will then be mailed to you.

How do I get a receipt for a canceled marker plate?   
You will need to provide us with your name and mailing address so we can send you the required Copy Records Request (form J-23). You will need to complete sections 2 and 3 on the form and the requester section, including your telephone number. Return form with the required fee of $20.

My insurance company has requested information concerning my Connecticut license number and the duration of the license in Connecticut. What do I do? 
Please provide your name and current mailing address and we will mail you the required Copy Records Request (form J-23). You will need to complete section 1 of the form, providing us with your name, date of birth and address as it appears on your Connecticut driver's license. You will also need to complete the requester section, including your telephone number. If you need the original date of issue, please state so in section three (miscellaneous request). Return the completed form with the required fee of $20. You must also include copies of two forms of identification as stated on the J-23 form. The information will then be mailed to you.
